fix: update per-patient collected in reports to use mhPaidAmount + copayment

All per-patient SQL queries now compute:
  collected = mhPaidAmount + copayment
  balance   = totalBilled - collected - adjustment

Fixes patient rows showing $0 collected while report total was correct.
