fix: Deltains claim selectors and dedicated fee schedule

Fixes the transaction-type radio button (was matching nothing; now
targets the real data-testid), and adds a Deltains fee schedule
(seeded from DDMA's prices) so Deltains claims stop falling back to
MassHealth pricing.
